In this rewarding role, you will provide physiotherapy services in the community with a focus on preventing hospital admissions and supporting recovery. Clinical areas include post-illness, falls, surgical interventions, as well as respiratory, cardiac, and pulmonary rehabilitation.
You will work with patients in acute or ED wards, Hospital in the Home (HiTH), and other community care settings, while collaborating with the Bathurst Physiotherapy Department, Medical Ward, Ambulatory Care, and HiTH teams.
Key Responsibilities:
Deliver physiotherapy care to patients across community and acute care settings
Minimise hospital admissions through effective post-illness and surgical interventions
Support rehabilitation in respiratory, cardiac, and pulmonary care
Collaborate closely with allied health professionals, medical teams, and community outreach services
Build long-term relationships with patients to improve health outcomes within their communities
About You:
Qualified Physiotherapist with current AHPRA registration
Experience in community, acute, or post-acute care preferred
Strong clinical reasoning and problem-solving skills
Ability to work independently and collaboratively within a multidisciplinary team
Commitment to delivering patient-centred care
