Reporting:Â This position collaborates closely with the rehabilitation team, interdisciplinary staff, and physicians to support comprehensive care planning.Â
Role OverviewÂ
- Evaluate residents and develop individualized physical therapy treatment plansÂ
- Implement evidence-based interventions to improve functional outcomesÂ
- Document resident progress accurately and in compliance with regulatory requirementsÂ
- Collaborate with interdisciplinary teams and physiciansÂ
- Supervise Physical Therapist Assistants and therapy aides, as applicableÂ
- Monitor clinical outcomes and contribute to departmental performanceÂ
What We’re Looking ForÂ
- Active California Physical Therapist license in good standingÂ
- Bachelor’s, Master’s, or Doctorate degree in Physical TherapyÂ
- New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferredÂ
- Knowledge of PDPM, Medicare, and skilled nursing facility regulationsÂ
- Strong leadership, communication, and organizational skillsÂ
- Commitment to resident-centered, high-quality careÂ
