Salary

Hourly Rate: $41.73 – $54.28/hour (dependent on experience)

Schedule: Full-time, Monday–Friday, 8:00 AM – 4:30 PM

Travel: Approximately 10% local travel for home visits and educational sessions

Job Description

Clinical Magnet is seeking a Physical Therapist (PT) for a direct hire role in Loveland, CO. This role involves evaluating and treating older adults and geriatric patients at acute, restorative, or maintenance levels of care, ensuring patients achieve maximum functional independence.

Key Responsibilities:

Conduct thorough evaluations and develop individualized physical therapy plans

Integrate therapy plans into physician-directed Plan of Care (POC)

Collaborate with interdisciplinary teams, caregivers, and facility staff

Educate patients and families on home exercise programs, DME, and safety

Supervise and delegate tasks to PTAs and Rehab Aides in accordance with state regulations

Conduct occasional home visits and educational sessions (approx. 10% of time)

Document all therapy services and patient progress accurately

Qualifications

Bachelor’s Degree in Physical Therapy (Master’s or Doctorate preferred)

Graduate of an accredited Physical Therapy program

Active PT license in the state of practice

Minimum 1 year of experience working with frail or elderly populations

Strong communication and collaboration skills, especially in interdisciplinary care teams
