Artemis Healthcare Partners is seeking a dedicated and passionate professional to join our client's team!

Position: Physical Therapist
Location: Sonoma County
Employment Type: Direct-Hire, Permanent
Setting: Home Health & Hospice
Pay: 0-3 years of Home Health experience: $2700/wk. 6 years of Home Health experience: $3300/wk
Shift: Full-Time

Qualifications:
– Doctor of Physical Therapy or Baccalaureate/Master’s in PT (licensed before January 2016)
– CA PT License
– CPR Certification
– Valid CA Driver’s License and Auto Insurance.
– 1+ year of Home Health experience
– EMR experience (Homebase Homecare EMR preferred, not required)

Key Duties:
– Provides physical therapy services based on a physician's plan of care and state regulations.
– Assesses and evaluates patients' therapeutic, rehabilitative, and functional status.
– Evaluates the home environment and assesses muscle strength, mobility, gait, and range of motion (ROM).
– Directs physical therapy treatment and prioritizes patient needs for therapy.
– Instructs patients and caregivers on the use and care of therapeutic appliances.
– Reports patients' reactions to treatments and any changes in their condition to the physician.
– Initiates therapy programs and educates family members or other personnel involved in patient care.
– Identifies and refers patients and families to additional home health services as needed.
– Prepares clinical progress summaries based on goal attainment and participates in discharge planning.
– Supervises physical therapy assistants and provides education to nursing staff as needed, adhering to attendance and reliability guidelines.
