Reporting:Â This position collaborates closely with the rehabilitation team, interdisciplinary staff, residents, and families to support comprehensive care planning.Â
Role OverviewÂ
- Evaluate residents and develop individualized occupational therapy treatment plansÂ
- Implement evidence-based interventions to support functional independenceÂ
- Document resident progress accurately and timely in accordance with regulatory requirementsÂ
- Collaborate with interdisciplinary teams to optimize resident careÂ
- Participate in care conferences and family meetings, as applicableÂ
- Mentor Occupational Therapy Assistants and support staff, as applicableÂ
What We’re Looking ForÂ
- Active California Occupational Therapist license in good standingÂ
- Bachelor’s or master's degree in occupational therapyÂ
- New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferredÂ
- Knowledge of PDPM, Medicare, and skilled nursing facility regulationsÂ
- Strong communication, organizational, and teamwork skillsÂ
- Commitment to resident-centered, high-quality careÂ
