$80.00 – $120.00 per visit

We are seeking a dedicated and compassionate Certified Home Health Occupational Therapist to join our team. In this role, you will be responsible for providing high-quality occupational therapy services to patients in their homes, skilled nursing facilities, and outpatient settings. You will work closely with individuals of all ages, including geriatrics and pediatrics, to assist them in achieving their rehabilitation goals and improving their quality of life. Your expertise in patient care, discharge planning, and case management will be essential in developing effective care plans tailored to each patient's unique needs.

Responsibilities

• Assess patients' needs and develop individualized treatment plans that focus on improving daily living activities (ADLs)
• Provide therapeutic interventions to enhance patients' physical, emotional, and social well-being
• Collaborate with multidisciplinary teams to ensure comprehensive patient care
• Conduct discharge planning to facilitate smooth transitions from inpatient to home health or outpatient settings
• Document patient progress and modify treatment plans as necessary based on individual responses
• Educate patients and families about therapeutic techniques and strategies for ongoing improvement
• Work with individuals with disabilities, autism, and various medical conditions to promote independence
• Utilize knowledge of anatomy, physiology, medical terminology, and occupational health in patient assessments
• Engage in early intervention strategies for pediatric patients to support developmental milestones
• Maintain compliance with all regulatory requirements and best practices within the field

Experience
The ideal candidate will possess the following skills and experience:

• Proven experience in home health, outpatient clinics, skilled nursing facilities, or acute care settings
• Familiarity with geriatrics, pediatrics, case management, and working with individuals with disabilities
• Knowledge of speech therapy techniques is a plus
• Experience in inpatient settings or ICU environments is beneficial
• Strong understanding of care plans and discharge planning processes
• Ability to work collaboratively within a team while demonstrating excellent communication skills
