*Position Overview*
* A License Skilled Home Care Agency located in East Longmeadow, MA is seeking a Certified Occupational Therapist to join the team. We provide high quality skilled home health care nursing services, Physical therapy and occupational therapy to Medicare and CCA patients. We are searching for a qualified and experienced occupational Therapist to join our team
*Responsibilities*
*Responsibilities for this Certified Occupational therapist include:*
* Make home visits to clients in homes.
* Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing and safety assessments.
* Develop and implement appropriate individualized care plans.
* Continually assess and revise the care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
* Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the patient’s health, safety and independent living.
* Accurately document observations, interventions and evaluations pertaining to patient care management and services.
*Qualifications*
*Qualifications include:*
* A current license as an Occupational Therapist in Massachusetts
* A minimum of one year of recent work experience as an Occupational Therapist Assistant.
* Ability to work independently and manage time effectively
* Strong interpersonal skills, organizational skills and problem solving skills
