The Licensed Occupational Therapist in Home Health is responsible for delivering personalized therapy services to patients within their homes. This role focuses on assessing patients' needs, planning and implementing treatment strategies, and ensuring progress towards improving their functional abilities and quality of life.

- Conduct thorough patient assessments to determine functional abilities and needs
- Develop and implement individualized treatment plans
- Perform home visits to provide hands-on therapeutic interventions
- Document patient progress and update care plans accordingly
- Coordinate care with other healthcare professionals and family members
- Educate patients and families on therapy goals and self-management techniques
- Recommend adaptive equipment to support patient independence
- Train family members to assist with therapeutic activities
