*Overview*
Join our dynamic healthcare team as a Certified Occupational Therapy Assistant (COTA) and/or Occupational Therapist (OT), where your expertise will directly enhance patient recovery and independence. In this vital role, you will deliver personalized therapy services traveling to assisted living communities and home environments locally. You will deliver skilled services via home health cases with our partnered HH agencies. Your energetic approach and compassionate care will empower our geriatric patients with acute or chronic conditionsto achieve their highest functional potential. *Duties*
* Implement and modify individualized care plans in collaboration with occupational therapists and healthcare teams to meet each patient’s unique needs.
* Assist patients with Activities of Daily Living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence.
* Conduct therapeutic interventions in home health visits, while focusing on improving motor skills, cognitive functions, and overall well-being.
* Utilize your physiology and anatomy knowledge to assess patient progress and adapt treatment strategies accordingly.
* Document patient care activities accurately in medical records, ensuring compliance with healthcare regulations and facility policies.
* Educate patients and their families on therapeutic exercises, safety techniques, and adaptive equipment to support ongoing recovery at home or in community settings.
* Collaborate with multidisciplinary teams to develop comprehensive care plans that incorporate medical terminology understanding and occupational health principles.
*Qualifications*
* Valid licensure or certification as a Certified Occupational Therapy Assistant (COTA) or licensed Occupational Therapist (OT).
* Prior experience working in skilled nursing facilities, inpatient units, outpatient clinics, or home health environments is highly preferred.
* Demonstrated ability to assist with ADLs and develop effective care plans tailored to individual patient needs.
* Familiarity with geriatrics, acute care protocols, and outpatient therapy practices.
* Excellent patient care skills combined with a solid understanding of physiology, anatomy, medical terminology, and occupational health principles.
* Ability to work independently in home or assisted living settings while maintaining excellent communication within healthcare teams. Pay: $48.00 – $65.00 per hour
