CPFT is seeking a Senior Physiotherapist to join their Intermediate Care Team, providing patient-focused, community-based rehabilitation to support early discharge from hospital using the “home first” model. The role offers variety and the chance to make a tangible difference in patients’ lives.
Key Responsibilities:
Conduct comprehensive, holistic therapy assessments and rehabilitation in patients’ homes within a 4-week pathway
Hold clinical responsibility for patient care and oversee Therapy Practitioners in the team
Prioritise workload for yourself and Therapy Assistants
Collaborate proactively with the wider multidisciplinary team to ensure patient flow and safety
Assist in the development of Integrated Care Workers and Therapy Practitioners to support clinical delivery
Candidate Requirements
Qualified and HCPC-registered Physiotherapist
Experience in community-based rehabilitation, ideally with complex or diverse caseloads
Leadership skills to supervise and develop junior therapy staff
Strong communication and collaboration skills within multidisciplinary teams
