Salary
Annual Salary: $95,000 – $135,000
Shift: Full-Time Days, 8:30 AM – 5:00 PM, Monday–Friday
Location: Lexington County, SC
Job Description
As a Home Health Physical Therapist, you’ll provide individualized therapy in patients’ homes, helping them regain strength, manage pain, and achieve their personal goals. You will work collaboratively with physicians and interdisciplinary teams to develop and implement treatment plans that are tailored to each patient’s daily life.
Key Responsibilities:
Evaluate patient function, performance, and equipment needs in collaboration with physicians
Develop personalized treatment plans with short- and long-term goals
Select appropriate interventions, frequency, intensity, and duration of therapy services
Conduct initial and ongoing comprehensive assessments
Provide physical therapy services as ordered in the plan of care
Participate in interdisciplinary patient assessments and care coordination
Qualifications
Current South Carolina Physical Therapy (PT) license
New graduates are welcome to apply
