Job Description
Committed to Caring is seeking a Physical Therapist (PT) to provide home health therapy services across multiple Texas locations. The PT will administer individualized therapy plans in patients’ homes, collaborate with caregivers and interdisciplinary teams, and supervise therapy assistants as needed.
Key Responsibilities:
Administer physical therapy services according to physician orders and plan of care
Develop and evaluate individualized plans of care with patients and caregivers
Participate in patient, caregiver, and family counseling and education
Supervise Physical Therapy Assistants and Home Health Aides as appropriate
Communicate with physicians and healthcare practitioners regarding patient care
Contribute to the Quality Assessment and Performance Improvement (QAPI) program
Travel to patient homes across assigned territories (100% travel required)
Qualifications
Licensed Physical Therapist in the state(s) of practice
Minimum 2 years of experience (Home Health preferred)
CPR certification (American Heart Association or Red Cross)
Strong verbal, written, and interpersonal communication skills
Demonstrated flexibility, initiative, and ability to work independently
Current valid driver’s license, reliable transportation, and auto liability insurance
High School Diploma or GED; graduate from an accredited PT program
Physical Requirements & Working Conditions
Travel to and work in patient homes
Physical demands include kneeling, squatting, reaching, twisting, climbing, walking, and assisting patients up to 100 lbs
Exposure to temperature/humidity changes, body fluids, odors, and other environmental conditions
Ability to perform duties with or without adaptive devices for sight/hearing
