Job Description
Trilogy Home Healthcare is seeking a Physical Therapist (PT) to provide one-on-one therapy services to patients in their homes. The PT will help patients improve mobility, strength, balance, and independence while ensuring a safe and supportive home environment.
Key Responsibilities:
Conduct individualized physical therapy sessions in patients’ homes
Assess and improve mobility, strength, and balance
Develop and manage care goals that promote recovery and independence
Evaluate the home environment for safety and accessibility
Document patient progress and communicate with interdisciplinary teams
Qualifications
Graduate of an accredited Physical Therapy program
Current Florida PT license
Valid Florida driver’s license, active vehicle insurance, and clean driving record
Minimum 1 year of experience (Home Health experience preferred)
Strong clinical judgment and commitment to patient care and outcomes
