Salary

Full-Time Salary: $80,000–$120,000 annually (with weekly bonuses)

Part-Time Compensation: Paid per visit

Additional Earnings: Weekly bonus opportunities

Mileage Reimbursement: Provided

Pay Schedule: Weekly pay

Job Description

Interim HealthCare is seeking a Home Health Physical Therapist to provide skilled therapy services in patients’ homes. As a PT, you will play a crucial role in improving mobility, independence, and quality of life through personalized treatment plans and one-on-one care.

Key responsibilities include:

Performing comprehensive in-home PT evaluations

Developing individualized treatment plans aligned with patient goals and physician orders

Delivering evidence-based interventions to improve mobility, strength, and balance

Educating patients, caregivers, and families on safety, home exercise programs, and equipment use

Monitoring progress and documenting all care accurately and promptly

Collaborating with physicians, nurses, and the interdisciplinary team

Participating in case conferences and professional development

Ensuring compliance with state practice acts, agency policies, and federal regulations

This role offers high autonomy, flexibility, and meaningful one-on-one patient impact through home-based care.

Qualifications

Licensed Physical Therapist (PT) in the state of Michigan

Minimum 1 year of clinical experience (home health experience within the last 5 years preferred)

CPR certification

Valid driver’s license, reliable transportation, and proof of auto insurance

Strong communication, clinical reasoning, and time-management skills

Compassionate, patient-centered approach and ability to work independently
