Occupational Therapist (Home Health)
We are seeking a compassionate and skilled Occupational Therapist to provide in-home therapy services to patients as prescribed by the attending physician. This role focuses on helping patients regain independence and improve their physical and cognitive functioning, while collaborating closely with physicians, families, and the care team.

Required
- Bachelor’s degree or higher
- Occupational Medicine (1+ years)
- Social Work (2+ years)
- Valid Occupational Therapist, Registered (OTR) in the state of Michigan (MI)
- Valid Regular Driver License (DL)
- ANY of the following valid licenses/certifications:

- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Pediatric Advanced Life Support (AHA PALS)
- Could be a new grad or someone with less working experience as an OT.
Key Responsibilities
- Conduct comprehensive occupational therapy evaluations to assess patients’ functional abilities and develop individualized treatment plans with measurable short- and long-term goals
- Implement physician-approved therapy programs aimed at improving independence and quality of life
- Educate and guide patients and family members on therapeutic exercises, adaptive techniques, and self-care strategies
- Train patients and caregivers in the use of orthotic and prosthetic devices; design and fabricate adaptive equipment as needed
- Perform ongoing functional and cognitive reassessments and adjust treatment plans accordingly
- Collaborate with patients and families to ensure understanding and adherence to the full treatment program
- Maintain accurate, timely clinical documentation in compliance with agency policies
- Communicate regularly with physicians regarding patient progress and recommend modifications to care plans when appropriate
- Coordinate outpatient occupational therapy services when needs cannot be met in the home setting
- Provide consultation and training to home health team members on therapy techniques, equipment, and activity programs
- Prepare and submit required reports to physicians, payers, and agency leadership
- Stay current with professional best practices through continuing education and in-service training
- Participate in team meetings, case conferences, and agency development initiatives
- Perform other duties as assigned
Schedule
- Monday – Friday, 8:00 AM – 5:00 PM
$55-65 PER/VIST/HOURLY
