Job description:
Job Title: Nurse Practitioner
Reports to: Clinical – Physicians
Summary: To meet the needs of the patients and providers of Fox Valley Pulmonary Medicine
Essential Job Duties:
Provides diagnosis and treatment of pulmonary, critical care and or sleep acute and chronic illnesses; including taking histories, conducting physical examinations and monitoring treatments in clinic and hospital setting as assigned
Provides health education to patients about ways to improve, promote and maintain their health status
Maintains accurate records, medication lists and documentation of care and follow up for continuity of care and administrative purposes for reimbursement of services
Maintains competence in clinical practice
Patient comfort, dignity and confidentiality are maintained at all times
Calls are returned in a timely manner
Communication with physicians, pharmacies, patients, etc is complete, accurate, efficient, timely and respectful
Demonstrates responsibility for individual performance and efficient utilization of practice resources
Serves as a valuable part of the patient care team
Maintains proficiency in computer skills
Other duties as assigned

Requirements:
Education:
Graduate of an accredited school of Advanced Practice Nurse Practitioner
Lic / Cert: Current Wisconsin R. N. and APNP or FNP license
Experience: One year in a clinic and Sleep Medicine experience preferred.
